ABSTRACT. Retroperitoneal hemorrhage is a well-recognized but relatively rare condition with a higher incidence in patients receiving oral anticoagulant therapy. It is most commonly seen as a complication of femoral artery catheterization and pelvic or lumbar trauma. It can happen spontaneously, without obvious precipitating factors, resulting in a spontaneous retroperitoneal hematoma, commonly seen in patients on hemodialysis or are anticoagulated. We present a case of massive retroperitoneal bleeding with an uncommon etiology, spontaneous erosion of the left renal vein by a staghorn calculus. Surgical exploration was mandatory because of the extension of bleeding and nephrectomy the only intraoperative choice. pp. 37–46
